Program Guidelines
- Patient’s portion of the bill is due the day of service.
- Cannot be used with another dental plan, insurance plan, or financing program.
- No refunds of plan fee will be issued at any time if the participant decides not to utilize the dental plan.
- NON-REFUNDABLE
- There will be a $50 reinstatement fee if your plan lapses.
Exclusions and Limitations
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan, dental insurance or financing program such as CareCredit.
- For treatment which, in the sole opinion of our doctors, lies outside the realm of their capability.
- For referrals to specialists.
- For hospital charges of any kind.
- For costs of dental care covered under automobile medical or injuries under worker’s compensation.

* The dual plan may be used for parent/child or spouse/spouse.
** The family plan includes family members and children to age 23 either living at home or enrolled full-time in school.
Fees must be paid at the time services are started. Fees not paid at this time are billed at prevailing usual and cutsomary fees. Plan fees are valid only when paid while enrolled. Family members must reside in the same household. This is not an insurance product.
